Go to the Student Profile, scroll down to the Medical section and click +Add.
Select Medical Institution Relationship.
Enter the information and click the green Add Relationship button.
If you can't select the right institution, follow this article to add a new Medical Institution. This will then appear in the dropdown when adding a new Medical Institution Relationship.
